- Form W-4
- I-9 Employment Eligibility Form
- Applicant Disclosure
- Emergency Medical Information
- Immunization History
- Retirement Status
- Direct Deposit enrollment
- Fingerprint Card and Instructions
The following are required before you can begin Substitute Teaching:
- All forms listed above
- Copy of Washington State Certification.
- Fingerprint submission or Verification.
- A copy of your Drivers License and Social Security Card.
